Proof of Vaccination Portal (the "Portal")

  1. Introduction
    As part of the large-scale vaccination project, the Government of Québec, in conjunction with the Ministère de la Santé et des Services sociaux, (collectively "Government of Québec") automatically provides the public ("User") with a QR Code and offers related promotions such as contests. For these purposes, the Government of Québec collects information for identification purposes with Québec Vaccination Registry.

    The Government of Québec has established and implemented internal policies and procedures to adequately protect personal information that is transacted through its infrastructures. The Government of Québec makes its employees aware of the importance of protecting personal information by requiring training on this subject and by issuing directives from time to time outlining their role and obligations in this regard.

  2. Applicable Laws
    This policy complies with the laws set out in the Personal Information Protection and Electronic Documents Act (PIPEDA). In addition, the collection, use, communication, and retention of your personal information is subject to sections 19 to 28 of the Act respecting health services and social services (R.S.Q., c. S-4.2), the Act respecting access to documents held by public bodies and the protection of personal information (R.S.Q., c. A-2.1) or the Act respecting the protection of personal information in the private sector (R.S.Q., c. P-39.1).

  3. Consent
    Subject to the exceptions and requirements of applicable laws, the Government of Québec will not communicate or disclose your personal information to any other third party without your free, informed and express consent. Users agree that by using the Portal, they consent to:
    1. The terms and conditions set forth in this Privacy Policy;
    2. The collection, use, and retention of the data listed in this policy;
    3. The sharing of data with partners authorized by the Québec government for the purposes of proof of vaccination;
    4. Obtaining data from partners authorized by the Government of Québec for the purposes of proof of vaccination.

  4. Personal data that the Government of Québec collects
    Data collected automatically on the Portal
    When you use the Portal, it is possible that “cookies” automatically collect information about the device used to access the Portal. It is possible to disable cookies from your web browser.

    Data collected non-automatically on the Portal
    For the purpose of obtaining proof of vaccination, you consent to provide the following information, but not limited to:
    1. First and last name;
    2. Email address;
    3. Phone number;
    4. Health insurance number;
    5. Date of vaccination ;
    6. Gender of the User;
    7. User's region of residence;
    8. Region of vaccination site;
    9. Type of vaccine administered.

  5. How the Québec government uses personal data
    Personal information collected during the use of the Portal will be used only for the purposes specified in this policy. The Government of Québec does not sell your personal information to third parties. The data that the Government of Québec collects may be used for the following purposes:
    1. To provide proof in downloadable PDF format;
    2. To meet legal requirements;
    3. For statistical purposes and to improve the Portal; and
    4. To monitor the quality of customer service and prevent errors and fraud.

  6. With whom the Government of Québec shares personal data
    The Government of Québec discloses only to authorized employees and partners the User data they reasonably need to fulfill the purposes set forth in this policy. The Government of Québec will not sell or share your data with third parties for any other purpose, except as required by law or in connection with legal proceedings.

  7. How long does the Government of Québec keep personal data?
    The Government of Québec retains your personal information for as long as necessary for the purposes for which it was collected. The Government of Québec destroys this personal information in accordance with the Act respecting Access to documents held by public bodies and the Protection of personal information (R.S.Q. c. A-2.1) and its file retention policy. When the Government of Québec destroys your personal information, it takes the necessary measures to ensure its confidentiality and to ensure that no unauthorized person has access to it during the destruction process.

  8. Your Rights as a User
    You may, following the filing of a request and after reasonable notice, subject to any regulatory or contractual restrictions, consult or have corrected the personal information that the Government of Québec holds about you. The Government of Québec will provide you with such information within a reasonable period of time from the date of receipt of the written request. In certain specific circumstances, the Government of Québec may refuse to provide you with the requested information. Exceptions to your right of access include the fact that the information cannot be disclosed for legal or security reasons. You may also verify the accuracy and completeness of your personal information and, if necessary, request that it be amended. Any such request must be sent to the MSSS Access to Information Officer.

  9. Modifications
    This Privacy Policy may be amended from time to time to maintain compliance with the law and to reflect any changes to the Portal.

  10. Contact
    Any complaint, question, or request for access to or amendment of personal information should be directed to the MSSS Access to Information Officer.
