CarePath turns fragmented patient records into longitudinal, source-linked clinical intelligence so primary care teams can understand new and complex patients faster
TORONTO, September 30, 2026: Akinox will showcase CarePath at OMD Educates: Digital Health Conference on October 1 and 2, demonstrating how AI can help primary care teams spend less time reconstructing patient histories before the first visit.
New patients can arrive with years of clinical history spread across hundreds of pages of fragmented records. Before the first visit, clinicians and care teams may need to search those records, reconstruct the patient’s history and prepare the chart to understand the patient they are about to see.
CarePath is designed to shift that work from searching and reconstructing to reviewing and validating. It combines deterministic ingestion and extraction with AI-assisted synthesis to organize information across records and formats into a longitudinal clinical view. Clinicians can see the patient history over time, ask questions of the record and trace findings back to the underlying source for verification.
Rather than producing a standalone AI summary, CarePath creates a clinical information layer over the records organizations already have, giving clinicians an organized view they can explore, question and verify while keeping clinical judgment with the care team.

Canada Health Infoway supported a three-month evaluation of CarePath in primary care, focused on the use of AI-enabled clinical summaries for new patient onboarding and complex pre-visit preparation. Amplify Care participated as the evaluation partner.
At OMD Educates, Akinox will also preview an AI-assisted form preparation capability currently in development, designed to use information already contained in the patient record to prepare draft forms for clinician review.
